Your application should contain the following items:

  1. Completed checklist
  2. Updated CV
  3. Completed Application form
  4. Signed Learner Contract
  5. 2 letters of reference from your sponsors, one of which must come from a formal leader in your context who you report to or are accountable to such as: a chair, program director, chief, vice-dean, dean or other. A sponsor should be someone from your own network who is key to your ongoing development of, and dialogue about academic leadership. The reference letters can be included in the package or sent directly to Jackie McCaffrey, Program Manager by June 2, 2017.
