Your application should contain the following 7 items:
1. Completed checklist
2. Updated CV
3. Application form
4. The following items:

  • Current Teaching Philosophy Statement
  • A Summary of your Teaching Performance Ratings
  • A List of your Creative Professional Activities in Education, if applicable
  • A List of your Research in Education Activities, if applicable
    If you have a teaching dossier already prepared that explicitly identifies and describes these four areas, you are welcomed to submit it.

5. A letter of support form completed by your sponsor. A standard form has been developed for this purpose. A Sponsor is someone who will provide a commitment to protected time of a minimum of 1/2 day per week for 2 years as well as tuition fees for the course should your application be accepted. This is often the leader of the clinical/ hospital program or your work setting in which you participate.

6. Two reference letters must be provided which indicate why you should be selected for the Education Scholars Program. One of these letters must come from your Academic Department Chair, their designate or equivalent academic leader. This reference letter is to ensure that they are aware of your interest and will be committed to supporting you as an educator in your academic environment. In some circumstances, your Department Chair and Sponsor may be the same person.

The second reference letter can come from someone of your choosing. They should be aware of your abilities and potential as an educator. Many people choose their sponsor for this letter but that is not a requirement.

7. A signed consent form from yourself acknowledging your understanding that program participants will be expected to regularly attend all classes and participate in the ongoing evaluation of the program.
